Participants may start monthly - quarterly, semi-annual, or annual memberships at any time. 21-day challenges are offerend quarterly. New members may attend only 1 per year.

Please ignore the 10:00 AM times for Tuesday - Friday. By selecting the time above on Monday you'll go onto the roster. This service above in not complete.
Agreement- Consideration of Nick Jarosh LLC, accepting the participant(s) and providing the instruction designated above, I agree to pay the tuition due in accordance with this Agreement. I understand that the tuition and service package provided for in this agreement represents a commitment outlined by selecting the corresponding package above and that such tuition will not be reduced or refunded with the lone expect of injury outlined below. I acknowledge that a $30 fee will be charged for each late payment, dishonored check or electronic funds transfer, and declined credit card debit.
Paid-in-Full agreements and Injury/Illness: All paid in full memberships of 3,6,12 month are non-refundable or not ransferable.
Prior notice is required to hold/suspend a membership. Notice may be submitted by mailing written notice to Nick Jarosh LLC through E-mail (Nick.Jarosh@GMAIL.COM) or through mail 405 SE Magazine RD, Ankeny, IA 50021
I understand that the payable amount may include tuition and materials that may not be returned after use. I understand that I may regularly attend classes only at the locations indicated and may only use the facility during scheduled instruction times during which I must participate in a group fitness class. I agree to comply with the program guidelines and posted rules. As further consideration for Nick Jarosh Fitness accepting the participant(s) and providing the instruction designated above, I represent that the participant(s) designated above is (are) a)fully capable of participating in fitness cardiovascular and group strength training classes, b) that the health of the participant (s) designated above have been evaluated by a qualified physician and c) that the participant has(have) not been prohibited from any type of exercise or physical activity.
